Consent Form
1. Collection of your personal information
In due consideration of Article 15 of the Personal Information Protection Act of the Republic
of Korea, the ASEAN-Korea Centre (hereinafter referred to as “AKC”), the Duy Tan University
(hereinafter referred to as “DTU”), and Hankuk University of Foreign Studies (hereinafter
referred to as “HUFS”) hereby request your consent to col ect and/or use your personal
information.
Type of information to be col ected, col ection method, purpose of col ection/use, and the
period of retention/use are as fol ows:
A. Type of Information Col ected and Col ection Method
- Application-related Information: (English/Korean) name, date of birth, gender, address,
nationality, email address, passport details, alien registration card details, mobile number,
name of school, major, country of school, date of (expected) graduation, language
proficiency, relevant experiences, areas of interests and skil s.
- Col ection Methods: Online application and document submission
B. Purpose of Col ection/Use: Application of the 2023 ASEAN-Korea Youth Network
Workshop
C. Period of Retention/Use: Prompt destruction 3 years after the date of col ection
D. You have the right to withhold consent to the col ection and/or use of your personal
information. If you withhold consent, you may not proceed in the application process.
2. Privacy and Intellectual Property
The AKC, DTU, and HUFS reserve the right to utilize al data submitted by individuals to
AKC, DTU, and HUFS and to col ect data in the form of documents, photographs or videos
during “2023 ASEAN-Korea Youth Network Workshop” (hereinafter referred to as “the
Workshop”) without limitations.
Al participants agree that no data, results, and materials for use in the Workshop wil be
disseminated by means of publications, reports, or any other form without the express written
consent from the AKC, DTU, and HUFS of which these products, data, results, and materials
are its exclusive property.
The AKC, DTU, and HUFS reserve the right to update and change information on the
Workshop without further notice.
3. Code of Conduct
Participants hereby certify that upon completion of the application process, al information
submitted is truthful and accurate. Participants understand that falsification of any
information provided wil disqualify him or her from participation in the Workshop, and
falsification of information or impersonation wil result in legal actions.
Participants agree that al materials produced for the Workshop are original work. Plagiarism
wil not be tolerated by the AKC, DTU, and HUFS, and should AKC, DTU, and HUFS
discover instances of plagiarism, the individual wil be disqualified from the program.
Participants are expected and obliged to obey the laws of the Republic of Korea and
Socialist Republic of Vietnam while participating in the Workshop.
Participants understand with ful assumption of risk and acknowledge that AKC, DTU, and
HUFS, their members, of icers, elected representatives, and employees do not assume any
responsibility for the conduct and activities of any participant outside of the scope of the
program. Al participants wil be asked to leave when in violation of the Workshop guidelines.
