Semester Withdrawal Form & LOA Questionnaire Students are required to submit both the Semester Withdrawal (Leave of Absence form) and SUNY Korea Questionnaire form.
*If you are applying for military leave, you should also submit themilitary enlistment letter along with the leave of absence forms.
